This is too vague of a question to really answer, but here are some tips:
Find out what format the game you're modding accepts. Use Google. Read the game CD/DVD. Poke around.
Find a tool that will convert your models to this format. Google for it, or write your own -- most model file formats are reasonably straightforward if you have decent math/coding skills.
Transfer it to the PS2. This might mean putting it on a UMCDR, your HDD, memcard, or tranferring it via ps2link or naplink. Depends on the game, whatever works.
All of this assumes the game you're talking about (you never mentioned which one) will accept mods. The PS2 is not a PC, and most games were not designed to handle mods, since there is really no way to get them for most players.
